Support Workers
Heart Link Supports provides reliable, compassionate support workers who assist with daily living, personal care, community participation, and more. Our workers are carefully matched to each participant to ensure the best possible fit. The right relationship makes all the difference.
We understand that inviting someone into your home and life takes trust. That's why every support worker we place is thoroughly screened, trained, and genuinely passionate about making a positive impact.

Cleaning Services
Our NDIS-funded cleaning service helps participants maintain a safe, clean, and comfortable home environment. We understand that for many people, household tasks can be challenging. There's nothing to be embarrassed about. We're here to help without judgment.
Our cleaning team is friendly, professional, and respectful of your home and your privacy. We work around your schedule and preferences so that you always feel in control.
A clean home is about so much more than appearances. It's about your health, your wellbeing, and your sense of dignity. We take that seriously.

Support Coordination / Recovery Coaching
Understanding your NDIS plan can feel overwhelming. There's a lot to take in and it can be hard to know where to start. Our support coordinators work alongside you to help you understand your funding, connect with the right providers, and get the most from your plan.
We don't just tell you what's possible. We work with you to make it happen. Whether you're implementing your plan for the first time or navigating a plan review, we're here to guide you every step of the way.
Through recovery coaching, we also provide dedicated support for people living with a psychosocial disability. We work with you to build your understanding of the NDIS, develop skills and resilience, and stay connected with the supports that matter most to your recovery journey.
If you ever hit a roadblock or face a crisis, your support coordinator is there to help you find solutions quickly and calmly.

Occupational Therapy / FCAs
A Functional Capacity Assessment (FCA) is completed by one of our occupational therapists to understand how a person's disability affects their everyday life and what supports they need. It's a warm, thorough process that helps identify a participant's strengths, support needs, and the services or funding that will best support their independence and goals.
An FCA looks at how you manage daily activities such as personal care, cooking, shopping, and getting around, and identifies any equipment, home modifications, or therapy that may help. From there, your therapist can recommend the right level of NDIS funding and supports for your situation.
FCAs also provide valuable evidence for NDIS plan reviews and new applications. A well-prepared assessment helps you clearly demonstrate your support needs and can make a significant difference to the funding you receive.
Throughout, our therapists take a person-centred approach. We work at your pace, in your own environment, to get a true picture of who you are and what matters to you.

Behaviour Support
Our positive behaviour support helps participants who experience behaviours of concern to live safely, comfortably, and with greater independence. We take the time to understand the whole person and what may be driving a behaviour, rather than focusing only on the behaviour itself.
Working closely with you, your family, and your support network, our practitioners develop practical, person-centred strategies that improve quality of life and reduce the need for restrictive practices over time.
Every plan is tailored to your goals and environment, and we support the people around you to feel confident putting those strategies into practice.
